Transportation Specialist
Job Description
Start Corporation is looking for a friendly and professional driver.
Minimum Requirements
High School diploma or GED equivalent.
Be over 18 years of age.
Possess a valid driver’s license
Current CPR certification
Prior experience preferred
Core Competencies
Knowledge of Patient Centered Medical Home Model (PCMH) and team based approach to care.
Ability to relate to the public, regardless of racial, ethnic and economic status.
Maintains awareness of potential risk to clients, staff and facilities and notifies administration as needed.
Completes all required training at initial hire, then as required by Administration (e.g. Crisis Intervention, Mental Illness, Non-Violent Practices, Workplace Violence etc).
Conducts themselves in a manner of professionalism that is conducive to patient care in an organized and efficient manner.
Job Duties/Skills Required
At minimum, maintains set clinic hours and available for extended hours as needed.
Communicate effectively with clinic staff, patients and families.
Consults with facility personnel and administrative field personnel, collects and analyzes data to identify needs for the assigned special services program within the particular service area.
Maintains a clear driving record in order to be able to drive company vehicles when necessary for transportation needs of those we serve.
Notifies administration of any mechanical issues or needs with regards to the vehicle.
Maintains all receipts and data in a confidential manner. Attends weekly and/or monthly staff meetings.
Take instructions from Administration regarding destinations and timelines, and ensure patients are properly seated with seat belt attached for transportation.
Facilitates related services for health center patient as appropriate with respect to their confidentiality and privacy.
Assist members of the medical staff in unloading patients in a safe manner.
Assist clients in safely boarding and exiting the vehicle (e.g. wheelchair safety).
Maintains daily log of vehicle mileage.
Maintains patient signed release for each patient transported.
Maintains log of routine inspections of vehicle, daily, weekly and monthly.
Ensures patients arrive in a timely manner for all clinic appointments.
Communicates with clinic staff regarding emergencies, traffic, and patients unable to keep their scheduled appointment.
Perform other related duties as requested.
