Case Manager/Case Coordinator (Healthy Start)
Job Description
Seeking Case manager who provides high quality care to case manage/coordinate women, infants, and fathers to reduce mortality rates in women and infants in our community. Serves as Liaison between medical providers and clients, to help foster effective communication and appropriate plan of care. Is responsible for managing patient workflow for maximum efficiency to ensure productivity is met.
Currently hiring for St John Parish, St James Parish, Assumption parish, and St Charles Parish. Main office located in Houma, must be willing to come 1-2 times a month and meet virtual weekly.
Minimum Requirements
Bachelor's Degree in public health, nursing, health promotion, psychology, social work, sociology, or counseling.

Job Duties/Skills Required
Review treatment plan with supervisor, implement instructions, processes, and protocols, and assist with ongoing patient assessment and re-evaluation of patient problems or needs.
Collect data, record, and report changes in patient conditions.
Prioritize data collection based on patients immediate needs. -provide quality care based on the whole person, including health, nutrition, clothing needs, recognizing each individual.
Educate patients, staff, and family members regarding overall treatment on pregnancy, early childhood, safe sleep, tobacco cessation, breastfeeding, and healthy nutrition before during and after pregnancy.
Provide emotional support to participant before, during, and after pregnancy.
Case manager will meet the patient in home, virtual, or community setting
